Molecular cloning of the genes encoding the muscarinic acetylcholine receptors has shwon that receptor subtypes classified on the basis of pharmacological properties are related polypeptides encoded by distinct genes. These studies have laso revealed the existence of novel muscarinic receptor subtypes. Functional analysis of each of the subtypes expressed in mammalian cells indicates that the different subtypes activate distinct biochemical pathways, a finding that explains the tissue‐specific physiological response elicited by the neurotransmitter, acetylcholine.

In the past, hypothesis testing in medicine has employed the paradigm of the repeatable experiment. In statistical hypothesis testing, an unbiased sample is drawn from a larger source population, and a calculated statistic is compared to a preassigned critical region, on the assumption that the comparison could be repeated an indefinite number of times. However, repeated experiments often cannot be performed on human beings, due to ethical or economic constraints. We describe a new paradigm for hypothesis testing which uses only (...) rearrangements of data present within the observed data set. The token swap test, based on this new paradigm, is applied to three data sets from cardiovascular pathology, and computational experiments suggest that the token swap test satisfies the Neyman Pearson condition. It has been demonstrated that death certificates do not accurately record the actual cause of death in up to one-fourth of cases, as determined from subsequent autopsy findings. The purpose of this study was to explore the use of natural language autopsy data bases as an automated quality assurance mechanism. We translated the account of the major process leading to death, or the primary diagnosis, from all 45,564 narrative autopsy reports obtained at The Johns Hopkins Hospital between May 28, 1889, (...) and June 30, 1987, into the hierarchical system of Medical Subject Heading (MeSH) titles. We obtained a total of 125,772 MeSH title translations, 1,563 of them distinct (average 2.8 per case), ranging in frequency from 6,029 occurrences of LUNG to 1 occurrence apiece of 357 MeSH titles. The natural-language-to-MeSH translations showed expected trends over the past century: fewer infectious diseases; more cardiovascular and neoplastic disease among adults; and more respiratory diseases and congenital malformations in the pediatric age group. The greater availability of autopsy documents in electronic form should increase the value of this resource for quality assurance. (shrink)

In this paper I shall venture into an area with which I am not very familiar and in which I feel far from confident; namely into phenomenology. My main motive is not to get away from standard, boring, methodological questions like those of induction and demarcation; but the conviction that a phenomenological account of the empirical basis forms a necessary complement to Popper's falsificationism. According to the latter, a scientific theory is a synthetic and universal, hence unverifiable proposition. In fact, (...) in order to be technologically useful, a scientific hypothesis must refer to future states-of-affairs; it ought therefore to remain unverified. But in order to be empirical, a theory must bear some kind of relation to factual statements. According to Popper, such a relation can only be one of potential conflict. Thus a theory T will be termed scientific if and only if T is logically incompatible with a so-called basic statement b, where b is both empirically verifiable and empirically falsifiable. In other words: T is scientific if it entails ¬b; where b, hence also ¬b, is an empirically decidable proposition. (shrink)

Moore maintains that, in principle, there is an objective answer to questions of right and wrong. More specifically, that a particular action cannot be both right and wrong, either at the same time or at different times. In this chapter and the next, Moore argues against theories that deny this latter proposition and thus reject the objectivity of moral judgments. Beginning with a critique of the thesis that when one asserts that an action is right or wrong, one is merely (...) asserting that one has a certain feeling towards it, this chapter focuses its critical fire on various attitudinal theories of ethics. This chapter and the one that follows analyze and elucidate the normative structure of utilitarianism. Although Moore did not consider himself a utilitarian, it becomes evident as the book proceeds that he accepts utilitarianism’s consequentialist account of right and wrong despite rejecting its hedonistic value theory. These opening chapters are a model of analytic exposition as Moore lays out utilitarianism’s theoretical commitments and contrasts various distinct but closely related normative theses. In “Libertarian Natural Rights,” Siegfried Van Duffel endeavors to illuminate shortcomings in libertarian defenses of natural‐rights theory. Noting that defenses based on freedom beg the question, Van Duffel explores whether libertarians can find salvation in the concept of the sovereignty of the will, and concludes that this approach leads to incoherence. But because his arguments ignore the actual moral basis of natural rights, they at best fell a straw man, not libertarianism. Theidentification of explicit virtue components inthe definition and therefore conception ofevidence-based medicine presents an importantstarting point in the connection between virtuetheories and medicine itself. This chapter is Moore’s most important discussion of the subject of free will. He distinguishes the question of whether right and wrong depend not on what we can do if we choose, but rather on what we can do in some more absolute sense, from the question of whether we ever could have done anything different from what we actually did do. He analyzes closely the ambiguities of ‘could have done’ and ‘could have chosen’. The Term ‘liberty’ invokes such universal respect that most modern political economists and moralists endeavour to find a conspicuous place for it somewhere in their systems or prescriptions. But in view of the innumerable senses of this term an insistence on some kind of definition prior to any discussion seems to be justified. For our present purposes attention to two particularly conflicting interpretations will be sufficient. These are sometimes called the ‘negative’ and the ‘positive’ notions of Liberty. According to the (...) ‘negative’ notion, my own liberty implies the reduction to a minimum of the deliberate interference of other human beings within the area in which I wish to act. Conversely the absence of liberty, or coercion, is regarded as undesirable because it amounts to the prevention by other persons of my doing what I want. On the other hand, the ‘positive’ sense of the word ‘liberty’ consists in the attainment of self-mastery, or, in other words, the release from the domination of ‘adverse’ influences. This ‘slavery’ from which men ‘liberate’ themselves is variously described to include slavery to ‘nature’, to ‘unbridled passions’, to ‘irrational impulses’, or simply slavery to one's ‘lower nature’. ‘Positive’ liberty is then identified with ‘self-realisation’ or an awakening into a conscious state of rationality. The fact that it is contended that such a state can often be attained only by the interference of other ‘rational’ persons who ‘liberate’ their fellow beings from their ‘irrationality’, brings this interpretation of liberty into open and striking conflict with liberty in the ‘negative’ sense.
